INSTRUCTIONS FOR THE GENERAL PETITION

 

Please read carefully.

 

What can this petition be used for:

  • May be used to drop a course past the deadline
  • Request a Withdrawal past the last day to withdraw with a "W"
  • Request a refund past the refund deadline
  • Request Academic Forgiveness (Fresh Start)

Guidelines:
  1. All exceptions must be requested in writing to the Associate Dean of Student Success before the last day of the quarter in which payment was made.
  2. The student will complete this General Petition application and must attach all required documentations. Applications without required documentations will not be reviewed.

Exceptions may be made for the following:
  1. Medical problems severe enough to prevent attending.
  2. U.S. Military Call to Duty
  3. Administrative Error
  4. Other unforseen situations

Documentations required:
  1. Medical Reasons - Health care provider must verify "first date you could not attend due to medical reasons". Must be on a health care provide letterhead with original signature.
  2. Military Orders - Copy of your Military orders showing date you were called to active duty.
  3. Administrative Error - Explanation of administrative error.
  4. Other Unforseen Situation - Documentation supporting your unforseen situation.


Tips for your explanation and documentations:
  • State your request clearly and add as much background information as possible.
  • Anything that supports your reasons can be documentation. The more the better.
  • Medical and military documentation must be on official letterhead with phone number to call for questions.
  • If your request claims you were misadvised or is an administrative error, faculty/staff will be contacted.
  • Keep a copy of your petition and all documentations for your records. Upon submitting this application, a copy of your answers will be show and you may save it for your records.
IMPORTANT: Make sure you explain in detail the reasons for your request and attach your supporting documents.
Applications with no documentations will not be reviewed.

Appeals - what if your request is denied? If your request is denied, you will be informed of the process for filing an appeal.
