Enrollment Services Forms

Most Enrollment Services forms are available for online submission or printable PDF options. 

• Online forms can be completed and submitted using any electronic or mobile device. 
• Printable PDF forms allow you to electronically enter your information, save, and email your request, or if you want to print the form. 

How to Submit Your Request/Form:

Printed or saved forms must be submitted to the Enrollment Services Office in Building 17 on the Lakewood Campus.

1. Email your completed form to studentrecords@cptc.edu. 
   • Email requests must be sent from the preferred email address listed on file. 
2. Drop off your completed form at the Enrollment Services Office with a valid photo ID.
