Enrollment Services Forms

Most Enrollment Services forms are available for online submission or printable PDF options. 

• Online forms can be completed and submitted entirely using any electronic or mobile device. 
• Printable PDF forms allow you to electronically enter your information, save, and email your request, or if you are wanting to print the form. 

How to Submit your Request/Form:

Printed or saved forms must be submitted to the Enrollment Services Office, located in Building 17 on the Lakewood Campus.

1. Email your completed form to studentrecords@cptc.edu.
2. Drop off your printed & completed form at the Enrollment Services Office.
3. Mail completed form to Enrollment Services, 4500 Steilacoom Blvd. SW, Lakewood WA 98499.
