Enrollment Services Forms

Most Enrollment Services forms are available for online submission or printable PDF options. 

• Online forms can be completed and submitted using any electronic or mobile device. 
• Printable PDF forms allow you to electronically enter your information, save, and email your request, or if you want to print the form. 

How to Submit Your Request/Form:

Printed or saved forms must be submitted to the Enrollment Services Office in Building 17 on the Lakewood Campus.

1. Email your completed form to studentrecords@cptc.edu. 
   • Email requests must be sent from the preferred email address listed on file. 
2. Drop off your completed form at the Enrollment Services Office.
3. Mail the completed form to Enrollment Services, 4500 Steilacoom Blvd. SW, Lakewood WA 98499.
